Guide to your First Appointment
Phone Introductions
We schedule a brief introductory phone call for new prospective patients. We do not charge for this introductory call. This is an opportunity to meet Lee Porter, Psych NP, the clinician, by phone and see if there is good potential for a therapeutic and productive relationship. It is also an opportunity to review practice clinical policies and answer questions. The clinical inquiry phone call is confidential and no patient record will be created. If we agree to move forward, you will receive a call back for the next available intake appointment.
Appointments and Scheduling
All appointments are held via a secure telemedicine system that you can access via a desktop computer, a tablet or mobile device connected to the Internet. You must have an Internet connection capable of streaming video to have a satisfactory telemedicine session.

Before Your First Appointment
We are a telemedicine practice so we collect all information needed online.
There will be several forms required to verify your identity, obtain payment, discuss consents for care, and detail your relevant medical history.
Identifying Information
First of all, we will need an email address, phone and your name and birth date to communicate. You use this information to log into our secure, cloud-based medical practice system.
We require a valid drivers license or government issued photo identification to confirm the identity of new patients.
Health Insurance Verification
Please note that if you wish to use health insurance or other third party payer for your appointment fee, the coverage must be verified before we will confirm your appointment.
To allow for review of the information you submitted, we require patients complete the intake forms at least three business days before their scheduled first appointment.
2023 update: We have current contracts with Blue Cross, Regence, PacificSource, Cigna, Aetna and others. However we are not contracted for all plans with these companies. Insurance coverage of telemedicine or virtual care is changing with the ending of state and federal emergency, pandemic related rules for coverage of mental healthcare via telemedicine. Check your plan to make sure virtual care is a benefit.

When signing in and completing patient intake forms, please include the front and back of your insurance card so we can contact your plan administrator to verify coverage.
Video Telehealth
PREPARING FOR YOUR FIRST VIDEO APPOINTMENT
For your telemedicine appointments, please make sure you are in a private location without distractions, and test your camera to make sure you are comfortable with your location and video presence.
Link to waiting room. You will be asked to sign in and let us know you are in the waiting room. Lee will join you for your private appointment once you sign in. Video sessions are secure and are not recorded.

Patient Portal
The patient portal is how we communicate privately with our patients. We use this to obtain and send you medical records, educational materials and to communicate with you about treatments, labs, prescriptions and other medicines. Your email and date of birth will be used to set up a personal account.
There is a payment module that allows you to safely pay for your appointment.
